ITS Order forms are available in the following formats:

HTML - Interactive forms you may complete via your Web browser. Some require you to print a Web page and mail or fax that page to the appropriate contact.

MS Word Interactive - Microsoft Word documents with interactive forms. You should:

• Download the form to your desktop
• Enter the information in Word and then print OR
• Print and complete by hand

If you are not able to view these MS Word documents, you may need to download a MS Word Viewer (if you don't have any version of Word) or a MS Word Converter (if you have an older version of Word). Go to the Microsoft Download Center and search for Word Converters and Viewers. Select the appropriate Converter or Viewer and follow the download instructions.

In Windows, right-click the form's link and select "Save target as...." In the dialog box, select a folder in which to save the document.

On a Mac, hold down the option key while clicking the form's link and in the dialog box select a folder in which to save the document.
